To assist people with life-limiting illness manage their symptoms and improve the quality of life, by providing comfort through relief of pain and other distressing symptoms. Comfort will also be provided.
Duties and responsibilities :
- Provide compassionate, high-quality palliative care to the poorest of the poor. Serve patients with empathy, respect, and passion.
- Be responsible for the management and delivery of palliative care in a community setting.
- Liaise with referral doctors on the patient's medical condition so that proper care can be administered.
- Participate in the planning, implementation and activities related to promoting the growth of APS and ADOPTs
- Conduct thorough financial assessments to identify and prioritise patients who are in need of free healthcare services.
- Communicate with patients and their families to discuss treatment options, goals of care, and advance care planning.
- Assist patients in making informed decisions about end-of-life care options and support them in achieving their preferences.
- Following the death of the patient, bereavement visits to the family should be initiated when necessary and further follow up arranged as appropriate.
Qualifications :
- Diploma/Degree in Nursing
- Registered with Malaysian Nursing Board and possess valid APC
- Area of specialty - Palliation
Skills and knowledge :
- Able to work independently in care management of patients
- Strong decision-making with problem solving skills.
- Able to drive with own transport.
